If you are unable to print out the forms from our website, please make sure to have important information with you such as:
- Address, phone number, email
- Current Insurance
- Pharmacy name and phone
- Name, address, phone number of your PCP or any doctors you see regularly

Every patient gets personalized care and attention, so we never rush or discharge patients without answering all questions, concerns and make sure each patient is at understanding of their situations. Please be patient if wait times get a little longer.
When you are done with your visit, do not forget to schedule your following appointment. Schedules do get booked rather quickly so it is always better to schedule in advance. We always call in advance to confirm your appointments. In the case you cannot make your scheduled appointment, you can always reschedule.
Due to high volume of patients, we only call with results if they are abnormal. If you are interested in knowing your results, you can always call the office.
- Blood work: 5 days
- PAP smears: 7-10 days
- Biopsy/Pathology: 7-10 days
- Radiology: 5-7 days after tests are preformed
*These are the average amount of days it usually takes
If you need medication refills, please call at least a week in advance. It is not guaranteed that you can call in and get refill the same day.
Emergencies
If you have an emergency, please call 911 or go to the nearest urgent care. For non-emergency matters, you may contact our office at 718-680-4800 during regular business hours. If you call after hours, you may leave a voicemail, and your call will be returned on the next business day.
Requesting Your Medical Records
To protect your privacy, all record requests must be in writing. You can submit them in person, by fax, by email, or through the secure contact form on “Contact” page.
Please include:
- your full name, date of birth,
- type of records,
- where to send them.
Processing time is typically 1–3 business days, depending on staff availability, department hours, and patient volume.
